<p>Provide case management -managing a caseload of clients and maintaining case files.</p>

<ol>
<li>Provide case management and support</li>
<ol>
<li>Completes case management assessment on all clients entering caseload. (Complete the first portion of client intakes, complete intake forms, and record on-going progress notes in the client file)</li>
<li>Interview and assess clients, using appropriate assessment tools</li>
<li>Observe, record and report on an individual’s functioning</li>
<li>Read and understand assessments, evaluations, observation, and use in developing treatment plan</li>
<li>Identify community resources and services for clients and coordinate provision of services</li>
<li>Conducts crisis intervention as necessary</li>
<li>Monitors and verifies services provided to each client on a monthly basis, determining quality and effectiveness of services provided</li>
<li>Establish effective working relationships with relevant community organization</li>
<li>Develops appropriate treatment/service plans with clients as the means for implementing appropriate services and developing effective alliances with clients</li>
<li>Meet with the Women At Risk staff weekly to discuss current clients</li>
</ol>
</ol>

<li>Co-facilitate educational groups and substance abuse treatment groups if he/she has the appropriate training, certification and supervision.</li>
<ol start="1">
<li>Understand continuum for understanding client motivation for changing substance abuse behavior</li>
<li>Understand basic group process—how people interact within a group</li>
<li>Firm grasp of material being communicated in the psycho-educational group.</li>
<li>Be mindful both of the group’s need and the specific needs of each member</li>
<li>Design learning experiences that actively engage the participants in the learning process fostering interaction among members, managing conflict that inevitably arises among members in a group environment, and helping clients take ownership for the group</li>
<li>Understand the range of consequences a client faces because of relapse. (e.g. criminal justice penalties, child protective services actions, welfare‐to‐work setbacks)</li>
</ol>

<li>Conduct new client intake and ensure all supporting documents are verified and on file</li>

<li>Interview clients for emergency assistance and determine eligibility</li>
<li>Maintain case management files for all clients</li>
<li>Monitor and document progress of clients.       Assemble and take apart client files</li>
<li>Primary person to enter demographic data into the Women At Risk computer database</li>
</ol>

<li>Other Duties As Assigned</li>
<ol start="1">
<li>Supervise childcare staff, and when there is not adequate coverage, the Case Manager will assume that responsibility</li>
<li>Provide crisis management and court advocacy as needed. May attend court and/or screen inmates at the Buncombe County Detention Center after having been trained in jail and court protocol, as needed, especially when the Court Liaison is unavailable to do so</li>
</ol>
</ol>
<p>&nbsp;</p>
<p>Requirements:</p>
<ul>
<li>Candidate must have previous experience working with offenders, substance abuse and trauma.  Knowledge of the criminal justice system is desirable</li>
<li>Bachelor&#8217;s degree from an accredited college or university<strong></strong></li>
<li>Minimum of two years experience in case management services</li>
<li>Motivated with sense of teamwork</li>
<li>Good communication and people skills</li>
<li>Possess cognitive, organizational, and emotional ability and maturity to deal effectively with multiple stressors, deadlines and difficult situations</li>
<li>Demonstrated skills in working with clients from various backgrounds and embraces</li>
<li>Homeward Bounds belief that services should be offered with respect, empathy, and in the spirit of hope and recovery</li>
<li>Demonstrated proficiency in MS Word, Excel, and Internet usage.</li>
</ul>

<p>Preferred:</p>
<p>Drug and Alcohol Certification or Licensure</p>
<p>&nbsp;</p>
<p>There are a number of program areas of Homeward Bound of Asheville.  It is critical for each member of any program area to be willing to work with all other programs, all staff members of those programs and all clients of those programs.  We all have a common goal of working with people to end homelessness in our community.</p>

<p>Part Time: Wednesday – Saturday: 6:30AM-1:30PM and Saturday  4:30PM-6:00PM.</p>

<p>POSITION PURPOSE:   To complement the housing teams of Homeward Bound of Asheville through initial intake and offering case management until individuals are housed.</p>

<p><span style="text-decoration: underline;">Duties and Responsibilities</span></p>
<ol>
<li>Facilitate communication and coordination between care/service/support providers making appropriate referrals to housing, mental health, substance abuse, and other supportive services
<ol>
<li>Educate the client about service options, available resources, and case management</li>
</ol>
</li>
<li>Empower the client to problem solve in order to achieve outcomes</li>
<li>Promote client self-advocacy and self-determination</li>
<li>Communicate regularly with other staff via email, incident reports and briefings
<ol>
<li>Effectively utilize case conferencing and supervision to support case management work</li>
</ol>
</li>
<li>Create a welcoming and safe environment for clients.
<ol>
<li>Respond to requests to store possessions in container room, receive sundries and towels for showering, use the telephone and receive mail</li>
<li>Use all contacts to develop relationship with the goal of housing all individuals who are ready</li>
<li>Maintain safety for clients and staff, negotiating disputes or calling the police as necessary</li>
</ol>
</li>
<li>Assure cleanliness of facility, cleaning bathrooms/showers and floors daily
<ol>
<li>Daily tracking and managing of participant information in HMIS database</li>
<li>Prepare paperwork in accordance with program guidelines</li>
</ol>
</li>
</ol>
<p>b.      Maintain participant files to ensure completeness of required documentation and      verifications.</p>

<li>Any reasonable activities needed to insure the wellbeing of clients of Homeward Bound of Asheville</li>

<ul>
<li>Previous experience working with individuals experiencing homelessness</li>
<li>Previous experience working with individuals with mental health and/or substance abuse needs</li>
<li>Bachelors degree in Social Work, Counseling or related field</li>
<li>Excellent verbal and written communication skills</li>
<li>Computer experience in word processing and databases</li>
<li>Understanding and appreciation of the Homeward Bound mission and style of service delivery.  Commitment to facilitating this mission through position.</li>
<li>Team player who has superior client skills as well as capacity to engage in critical thinking and analysis of program and services delivery</li>
</ul>
<p><span style="text-decoration: underline;">Preferred Qualifications</span></p>
<ul>
<li>Certified Substance Abuse Counselor (CSAC) or CSAC intern status</li>
<li>Qualified Mental Health Professional status</li>
</ul>

<p><a href="http://www.citizen-times.com/article/20130206/NEWS01/302060033/Community-mourns-death-Aimee-Buchanan">Read this tribute to her in today&#8217;s Asheville Citizen-Times</a>. </p>
<p>A memorial service for Aimee will be held on February 16th at 4 p.m. in the Anderson Auditorium at the Montreat Conference Center.</p>
